Huntington Beach Pop Warner Signup InformationPlease be sure to read this page to avoid any signup difficulties.

To register and pay online via credit card do the following:

Click Here to complete the online registration and pay
Come to a “Sign-Up Day” or “Event Day” with

a.      Proof of online payment from the online registration

b.      Printout of the membership agreement from the online registration

c.      Sign OEC contract which we will provide

d.      Weigh your child.

To register in person:

Complete form (one for each child) and bring with cash or check to one of the remaining “Sign-Up Days.” See dates below. Note – “Event Days” are too late!

Documents to be delivered to your Team Mom:

1. Original birth certificate and 1 copy of birth certificate (originals will be returned)

2. Proof of residency (electric bill, gas bill, or driver’s license)

3. Report cards for the 2011-2012 academic year, not just the last semester.

4. Physical form signed by physician and dated no earlier than 1/1/12

5. Small mug-shot photo of player(s)

Note: team moms may choose to use one of our “Event Days” to gather the above paperwork.

2012 HBPWF Event Days

Event Days are offered so players, parents, coaches, team moms and board members can meet each other, gather paperwork, and get a head start on the season. We encourage everyone to attend. Team moms and business managers may use these Event Days to gather the paperwork listed above. We realize that report cards may not be ready by this time so we urge you get these to team mom’s immediately upon arrival. Certification of each team book as well as administration of the Little Scholars program requires all report cards.
